The lorry is in lane two and additionally the article says that the car had just joined the motorway. I'm guessing that the lorry was in lane two to allow cars to join the motorway in lane one, the car joined lane one and moved over the to lane two without checking the blind spot. I see so many more car drivers doing this when they join the motorway than bad driving on the part of lorry drivers.

I may be wrong but this seems the most likely cause in this incident.

I concur. It is probably her fault that she drove into his path. Though the fact that he failed to stop for a mile really damns him.
