I'm intrigued as to how products made by two different manufacturers (Reckitt Benckiser and AstraZeneca) became mixed up in this way. I imagine there will be some red faces, and quite possibly rolling heads, at the plant where these are packaged (bit surprised it's not in-house TBH).

Contract manufacturer and/or packager is commonly outsourced by the licence holder.
Sounds like someone on the packaging line did not do the clearance and sign off correctly, big breach of GMP and I'm the MHRA will be going in like a hot knife through butter to do a nsty audit.

I think the stats make some suggestions.

1. Only 3 batches effected
2. Only 3 cases reported
3. All cases in the same area

It sounds to me like there were most probably not swapped at the batch level, but in the store. One store in South London. By one person, probably on the take.
