From the fact that the crane is strewn across the road and motorway makes me think that this was a crane failure. The operators are usually strapped to the platform in case they fall out, so he would have stood no chance if the crane failed.

It looks to have simply fallen over. I'd guess it was down to human error or neglect. No comfort to the dead and the grieving in speculation though.

It looks like any safety features to stop the crane being extended into unstable configuration were not followed. Still could be mechanical or operator failure.
