I always leave the car in gear, as well as applying the handbrake... And, even with the handbrake on, if the car is in gear, it will still lurch, if you turn on the ignition without putting it in neutral / dipping the clutch.

It will lurch, but if you release the keys, or fall out of the car as in this case, the engine will stall. It is an unfortunate incident, but I doubt it would have happened had the rules of the road been followed.

That is what I thought

Surely a 2 year old would have to be inside the car to start it purely because of distance to the socket

I would guess that to be able to start and then to be thrown out of the car she would be standing on the sill.
