Purely out of habit. The toilet roll holder in the house I grew up in was more if a tray with a serrated edge. You had to put the roll in that way or it wouldn't work.
A bit like the way cling film boxes have a serrated edge. It worked the same way.

Defnitiely B here. If I find A I have a habbit of changing it to B because it bugs me.

B is the way I've always done it. We used to have a loo roll holder that had a springed flap at the top. This would limit how much toilet roll you could use. It was designed to be used as B according to the photo on the box.

Interestingly, the Simpsons were marked down for having toilet roll in the overhang/A position (Home Sweet Homediddly-Dum-Doodily).

It's A in this house, presumably my dad thought it looked neater when fitting the holder. Frankly I just grab the roll that's sitting on top of the toilet lol.

In most modern flats the toilet sits quite close to the wall & the tend to forget about how far a toilet roll comes from the wall.

If you do B then the paper comes down and is in the wrong place to grab or sits on your leg.

A also looks better IMO
