Security testing is really neglected by so many companies and it shows, along with poor security measures.

In all honesty something of this scale could even be caused by something as small or simple as a buffer overrun into some injected malicious code. It's not difficult to do if you know what to target. even something like a malformed XML tag can bring a system down without proper input checking
